We also need an Author's Statement. Please print this page off on your computer, then fill in the form (below), sign it, and send to Linda Malnack, P.O. Box 426, Seahurst,
WA 98062. Since we're aiming for January 1st, we need to have statements in hand by December 24, 1999, if possible.
